How to set up Appointments
Call (817)-354-2888 to schedule an appointment.
Make sure you have your insurance information available to provide to us, so we can confirm your coverage. We have the list of insurance we accept below. If your insurance is not on the list, we are more than willing to find a resolution.

Monday-Thursday: 9:00am - 5:00pm
Friday: closed

Cancellation Policy
We are committed to providing exceptional care. Unfortunately, when one patient cancels without giving notice, they prevent another patient from being seen. Please call us 24 hours before your appointment time, to notify us of any changes or cancellations. To cancel Monday appointments, call us on Friday.
If prior cancellation is not given, you will be charged a missed appointment fee.
